Required Documentation
ReVive Surgicenter requires documentation to support financial hardship claims. This includes income verification and proof of expenses. All submissions are confidential and reviewed only by authorized administrative staff.
Review Timeline
Once we receive a completed application and all supporting documents, a determination will be made within 10 business days. All approvals or denials are made at our discretion, based on our hardship guidelines. Applicants will receive a written notice of the decision. If denied, you may reapply if your financial situation changes significantly. All financial hardship documentation is kept confidential and on file with our administrative team. We reserve the right to verify your ongoing eligibility at any time.

Completed forms and supporting documents must be submitted within 60 days of your service date.
Financial Collection Policy
ReVive Surgicenter accepts most major insurance plans and offers flexible payment options. Please speak with our team in advance to arrange payment. We collect all co-pays, co-insurance, and any portion of your deductible on or before the day of your procedure. A team member will walk you through your payment responsibilities beforehand. You’ll receive monthly billing statements for up to three months or until your balance is paid. For questions about your account, our billing department is available to assist you.
Healthcare Practitioners at ReVive Surgicenter
Several independent practitioners operate within our facility. These providers bill separately from ReVive Surgicenter.
Before your procedure, you should contact the specific provider directly to:
Request a cost estimate for their services.
Confirm their insurance participation.
Important: Even if ReVive Surgicenter accepts your insurance, these independent practitioners might not. You may receive separate bills — one from the surgeon and another from the surgery center.
